Occasionally, your child may be prescribed medication by a qualified medical practitioner.  If the medicine needs to be administered during the school day, parents/carers MUST complete the permission form which can be downloaded at the bottom of the page.  If you need a hard copy, please ask the school office.

All medicine must be in the original packaging, with the label attached, which clearly shows the child's name, the date of issue, the dosage and the number of days the medicine is to be administered.  Without this information or the form, we are not allowed to administer prescribed medication.
